To assist the City of Kawartha Lakes in following up your concern, please complete the following with appropriate details.

Concerns are confidential until such time as the complaintant may be asked to testify in support of this submission.

I do hereby lay and serve complaint with concerns for fire safety at the above noted property with respect  to the noted conditions that are occuring and may not conform to the Ontario Fire Code and/or By-Laws of the Corporation of the City of Kawartha Lakes.

I hereby further declare that if required I will attend and testify in support of this complaint at any Court of Law in the Province of Ontario
