Moments and Memories Submission Form

Mapping our stories enables us to preserve stories from our past, to share memories passed down through our families, and to tell others about the traditions and activities that are unique to Kawartha Lakes.

Using moments submitted by the community, we will not only continue to celebrate the exceptional intangible heritage of Kawartha Lakes, but perhaps uncover some really special memories.

I confirm that the information that I have included in this form is accurate and complete, to the best of my knowledge.

I confirm that the story and photograph(s) are my original content.

I hereby grant to The Corporation of the City of Kawartha Lakes (the ‘City’), its agents, employees, Council, licencees and assigns, and those acting under their direction and authority, the absolute right for as long and for as often as it may elect, to use and publish the story, the history of the story and the photograph enclosed in any medium including but not limited to internet, print, social media, magazines and posters (the ‘Publication’).

I understand that I will retain all copyright to the original story and/or photograph(s) supplied unless they are already a part of the public domain. I further understand that the City of the Kawartha Lakes will hold copyright to any future publications (i.e. blog posts, social media, etc.) for which they are used. Credit shall always be given to the donor regardless of the items being in the public domain or not.

I acknowledge and confirm that I will be paid no amount of compensation or receive any consideration for the use of the story, the history of the recipe and the photograph and specifically waive any right to make claim for such compensation.

I acknowledge that I am expressly waiving the right to inspect and approve the image that the City proposes to use as described herein for the proposed publication.

I further acknowledge that I have the right to withdraw my consent as set out above upon giving reasonable notice in writing to the Manager of Economic Development.

I confirm that by signing this Consent and Release form release the City and the person creating the image, using the story and the history of the recipe from any and all claims that I may have, including any claims of defamation or invasion of privacy.

By signing this form, you agree that you have read and fully understood its contents. You further agree that your consent and release are binding on all your heirs, legal representatives, successors and assigns.
